Carbon Health, a leading American healthcare provider, has announced the launch of an all-new diabetes program, which offers an "omnichannel" approach to managing the disease that combines at-home glucose monitoring (CGM) devices with ongoing access to a care team called Steady Health, who are available both virtually or in person.
Carbon Health acquired the virtual diabetes care platform, Steady Health, in 2021 with the goal of integrating its services into a comprehensive patient management service. Now the company has successfully incorporated the platform, allowing healthcare teams to closely monitor their patient's health via continuous CGM data feedback.
"Our pilot program proved patients with uncontrolled diabetes can achieve significant glycemic improvements by working with CGMs and a virtual multidisciplinary care team to provide continuous clinical feedback and support," said Dr. Calvin Wu, Medical Director of Virtual Diabetes Care and endocrinologist at Carbon Health.
